UCSCs liftover is a very nice tool to convert SNP position from hg18 to hg19.

But I think that you could get better and more precise answers if you specify your question.

You can use the Ensembl Assembly Converter for this:

http://www.ensembl.org/tools.html

Note that the official names of the most recent two human assemblies are NCBI36 and GRCh37 .... ;)
