Where can I find the positions and rsids of SNPs on the Illumina Omni 5M array?
The Illumina site doesn't have it, and (strangely) only allows access to support files for existing customers.

Seems the UCSC database has it:
http://hgdownload.cse.ucsc.edu/goldenPath/hg19/database/snpArrayIlluminaHumanOmni1_QuadRaw.txt.gz
The linked file has only 1.13 million SNPs...

This is omni1_quad, an earlier version, the difference is here http://www.illumina.com/Documents/products/datasheets/datasheet_gwas_roadmap.pdf. omni5 is the newest version

Did you try emailing illumina?
Yes. The response was that it's only available in "iControl DB" or the support files, which is restricted to Illumina customers.
Can't fathom why a company would make this information hard to find.
