Converting from chromosome pos to rsID

Hi,

I recently used the topmed server to impute my genotyping data, the output SNP ID given is now formated as:

CHR:POS:REF:ALT e.g, chr10:10709:G:A

Does anyone know a way of converting this information to the relevant rsID? I'm using GChr38.
