I've never personally experienced two different gene symbols for the same gene regardless of genomic feature, but one reason that you may get a different gene symbol between a gene's promoter and a gene's exon or intron is that there may be overlapping genes.

Something like:

|promoter gene A|--------intron gene A------======exon gene A========------------------------|

                         |promoter gene B|======exon gene B====--------------------------|
