I've been working on a free web tool for turning clinical free text into Human Phenotype Ontology (HPO) terms — useful when you need HPO IDs for Exomiser, LIRICAL, Phenopackets, or Matchmaker-style workflows but the chart is still in prose. HPO Explorer (https://hpoexplorer.com) runs the note to HPO matcher entirely in the browser — no install, no login, and the note text is not uploaded to a server. It handles negation, family history, and uncertainty, and flags conflicts when the same finding is affirmed in one place and denied in another.
