There's no single strategy that is appropriate for all biomarkers, since for some you might assay an RNA whereas for others it may make more sense to assay a protein or enzymatic activity or something like a lipid content. In general, then, the strategy is to measure something biologically meaningful in a sufficiently large number of patients and matched controls.
