Dataset of (symptoms -> disease) pairs per patient

Hi, I don't have any medical background, but I'm working on a system that might give you a 'probability' of you having a disease/condition based on your symptoms. Something like WebMD(except better I hope). What I need for this is a dataset that maps recorded instances of a patient reporting symptoms X with them being diagnosed with disease Y. Can you guys help me find this?

This is in the realm of electronic health records (EHR) - I have also designed a prototype system that can analyse these records and make a prediction in terms of diagnosis and prognosis.

I do not know of any publicly available dataset. The data is inherently private (to the patient); thus, only trusted researchers at trusted institutions would be able to acquire access. There is a general and slow push toward making such data more accessible to more researchers, though.
